APPLICANTS FOR THE PRACTICAL ROAD TEST MUST CANCEL 24 HOURS PRIOR TO THE DATE OF THE TEST. FAILURE
TO COMPLY WILL RESULT IN ANOTHER PAYMENT OF THE PRESCRIBED FEE.

APPLICANTS MUST PROVIDE THEIR OWN VEHICLE OR MAKE PROVISIONS PRIOR TO ATTENDING THE TEST AND BE
IN POSSESSION OF A VALID INSURANCE CERTIFICATE FOR THE VEHICLE BEING USED. THE INSURANCE SHOULD
STATE WHO IS COVERED BY THE INSURANCE.

YOU MUST HAVE THESE ITEMS TO DO THE TEST.
NO DAMAGED HELMETS OR ACCESSORIES WILL BE ACCEPTED. IF THESE RULES ARE NOT MET,
THE TEST WILL AUTOMATICALLY BE CANCELLED AND THE APPLICANT MUST REBOOK AT THEIR
EXPENSE.

Depending on the application being made, you will be required to provide digital copies of supporting
documents for the application to be processed and approved.
All fields marked with an asterisk (*) are required and each field has some help information. Please pay
attention to the correct format for dates and telephone numbers in particular.
Processing applications submitted online require backend processing and approval and customers are
required to allow at least 24 hours based on a Monday – Friday cycles to receive approval
notifications via email. Only then can a customer visit any of DVDL’s branches to process their
applications and pay the requisite fee for the desired service(s).
If an application is deemed to be urgent, then a customer may have to visit the branch and manually
complete a form to process such an application.
For your protection, once you start the application you will not be able to save it and return later on.
If you leave the application idle for 10 minutes, the page will expire and you may be returned to
the main page. If you click the 'Cancel Application' button at any stage all data entered will be
lost.
At the end of the process you will receive confirmation on screen that your application was successfully
lodged and a copy of the confirmation sent to your email for future reference. If you do not receive
your confirmation for the application and subsequent communication emails for your approval status in
your inbox, please check your email spam and junk folders. Ensure emails from dvdl.helpdesk@gov.ky are not
being blocked by your email spam or junk mail filter.
Your application will be based solely on the information you provide via the form and therefore, it is
imperative to ensure that you provide the most accurate details & associated documentation to support
your application. If there is an issue with your application, we may have to contact you for further
information, thereby delaying your approval beyond the 24 hours timeline.
All electronic documents must be PDF, JPG, GIF, TIF or PNG and no more than 2MB in size. Illegible
documents due to low image quality may result in delays to the processing of your applications.
