Application for a New Licence Plate

I do hereby apply for new number plates to be issued in respect of the above vehicle.

I certify that the original plate(s) have been lost, stolen or rendered illegible,
If lost or stolen a police report should be filed and a copy brought to the Department to support this application.

Note: A fee of $75.00 the application and $10.00 for replacement of EVR vehicle licence (coupon).
